Deadline: Please submit the Transportation Request Form for EVERY child in your family by August 1, 2025.

Submission: Once submitted, your information will go directly to our transportation department.

Address Update: If you have moved or plan to move before the school year starts, please update your address on this form with the correct information.

Guarantee: Completion of this form is necessary to guarantee transportation services.

Bus Route Information: We anticipate posting bus routes on our website, nilesschools.org, by mid-August to help you plan accordingly.
